Dominican Republic: Any entry or exit from the Dominican Republic is subject to a form that must be completed prior to departure or arrival. This form can be accessed from the following link:
https://eticket.migracion.gob.do/, and generates a QR code that must be presented to enter or leave the territory. This procedure is free of charge.
For travellers to Santo Domingo, it is advisable to select "Santo Domingo de Guzman" to identify the districts of the capital.
Note that it is no longer possible to complete this form at the airport ("papel azul" / "blue paper"). The Dominican authorities apply a fine of $80 if the QR code is not presented.
Jamaica : You must complete an Online Passenger Declaration Form, also known as an Electronic C5. Paper forms will no longer be accepted.

here.If the lead passenger is travelling with a spouse or children under the age of 18 years, they can complete one declaration form. For everyone else, they will need to complete their own form.
The online form is mandatory for all travellers to Jamaica, and we encourage customers to complete it prior to arrival.
US formalities
You must apply for an application for an entry permit to enter the territory called ESTA at least 72 hours before your departure date.
ESTA is required even for short transits and transfers in the U.S.

Foreign visitors to Zanzibar are now required to take out travel insurance upon arrival (up to 92 days' stay in the islands). All foreign visitors must take out mandatory travel insurance at a cost of US$44 per person directly from the website of the national insurer, Zanzibar Insurance Corporation, prior to arrival:
